Fertility claims assessed in April and May 2025 have been reprocessed to pay the new rates.
System changes were implemented in June 2025 to increase the rates for the following Fertility Fee Schedule Codes (FSCs):
- B600 – Complete Intra-Uterine Insemination (IUI) Cycle
- B601 – Incomplete Intra-Uterine Insemination (IUI) Cycle
- B603 – In Vitro Fertilization (IVF) Cycle Bundle 1
- B604 – In Vitro Fertilization (IVF) Cycle Bundle 2
- B606 – Funded Oocyte Fertility Preservation (FP) Cycle
- B607 – Funded Sperm Fertility Preservation (FP) Cycle
The new rates apply to services rendered on or after April 1, 2025. However, claims assessed before June 2025 were paid at an earlier rate.
